Private Group / Workplace Training / Skills Session

This form is for both our standard classes and for scheduling private skills sessions, which are available on an individual basis. Please be aware that skills sessions are specifically for participants in our “Blended Learning” program. To attend a skills session, it’s necessary to complete the online portion of your class beforehand. Kindly provide proof of completion by either printing your certificate or emailing it to us at office@lifelinechico.com.

If there are any specific mask requirements for your class, please inform us. Please note that the decision regarding mask requirements is made by the workplace or the students themselves.
